PERA sends multiple mailings during the year that are mailed to the address on PERA records. It is important to notify us of any name or address changes.
Benefit Recipient Name or Address Change
If you are receiving a direct deposit payment from us, you can change your address two ways:
Log into your myPERA account and update your address
Call the Member Service Center at 651-296-7460 or 1-800-652-9026, and we can change your address over the phone
If you receive a paper check payment, call our Member Service Center and we will mail a Payment Recipient Address Change Request form to you. We are unable to make this change over this phone due to a mailed payment.
Complete and sign the form (e-signatures not accepted)
Mail, fax, or drop off the form at our PERA office
Beneficiary Name or Address Change
If your beneficiary has a change of name or address, you can update your information by:
Logging into your myPERA account and completing the required fields
